A piece of iron became lodged in the jaw of a material processing plant. An employee climbed up the plant. Some material fell from the bottom of the jaw causing the counter-weight to rotate. This caused a belt to move and the employee's finger was pinned between a pulley and the belt. The employee's left ring finger was amputated to the first knuckle. The plant was locked out/tagged out at the time.
